Judy’s independent and personalized consultancy service can be broken into three areas:
- Initial Consultation and Funding options
- Assessment in your home – in-depth assessment to gain an understanding of you and your needs and environment and support networks and resources. We can then match appropriate needs to services and develop a personalised plan
- Care Plan Review and implementation – discussion and review with yourself and any care providers to make further adjustments to your situation
